RISK MANAGEMENT PAPER One of the outcomes of the changes affecting the health care industry is an increased demand for accountability. Health care providers are being called upon to demonstrate that they provide quality services, use resources wisely, and produce reasonable and predictable outcomes. The pressure for increased accountability comes from a variety of sources. Individual health care providers are responsible for providing evidence which demonstrates that they are able to and consistently do provide high quality, efficient and effective services. The future direction of the organization is charted based on this intense self-scrutinizing process. The results of the strategic planning process are summarized in a document called the strategic plan.

Because the strategic plan identifies who is responsible for carrying out each part and what the expected outcomes are, appraisals of performance is possible. Quality assurance programs evaluate the quality and appropriateness of client care and efforts to identify and resolve care-related problems. To determine whether quality care is given in a cost-efficient manner, there are utilization review programs. Program evaluation is an ongoing appraisal of the results of care.

Guidelines for implementation of all three types of measurement programs are presented. Managing a health company entails important functions that need to be utilized in its affairs. The health companys success depends on how the business and the company as a whole, is managed especially in the areas of planning, organizing, leading and controlling. In the area of planning, the management of the company streamlines policies and plans in running its business. In planning, strategic plans and priorities are identified while financial forecasting is done to ensure that its business finances can support the plans for implementation (List of Management Functions, 2005).

The situation I recently encountered was a management issue. There was obviously a confusion in the process and very little clarification and vague instructions during the turnover may have transpired. It seemed that the nurse in charge did not follow the instructions. In situations like these, segments of the organization must function to compliment the efforts of each other. As can be gleaned, obviously, this is a management issue. The incident happened on the 17 th of Jan.

It happened after the changing of nurses shift. The outgoing nurse gave the report to another nurse. There was obviously very little care exercised in the turnover of operations. The situation took some time before it was discovered and could have been fatal in the long run. The problem involved a one-day old client, borne at term 39 wks. He had an IV order of D 10 W at a rate 10 ml / hr .

However, the rate became 80 ml / hr for whatever reason. This was discovered about less than half an hour by another nurse after the changing of shift report. The nurse quickly stopped the IV right away. This happened at around 11: 30 and the baby's blood sugar already increased to 192 by 12: 00 noontime. They retook the baby blood sugar again at 1: 00 and it read as 103. Then it lowered to 28 at around 2: 00 PM and at 3: 00 it was 27.

They checked the serum and it was 27. Furthermore, the nurses administered a bolus at 6 cc and by 4: 00, the babys blood sugar was 84. More stabilization became apparent and leveled off. The figures said that at 5: 00 PM, it was a stable 85. Everybody was notified about the incident and the supervisor tried to analyze where the mistake occurred.

There were several excuses offered by the different nurses. One of the nurse practitioners commented that the infusion pump should have been alarmed when the IV was running at a rate of 80 ml / hr instead of 10 ml / hr . The two nurses involved here were not as attentive to the needs of the patient. The outgoing nurse should have turned over her responsibilities properly.

The incoming nurse, in the meantime, could have checked this earlier so that there would not have been such a big time gap before the mishap had been discovered. It was luck that the baby was doing fine after some interventions. His blood sugar stabilized and there were no serious consequences that occurred. One of the nurses told me that this type of incident is very rare at Kaiser. Yet, it could have been prevented. Deciding the number of staff required for a healthcare system and services are studied; coming up with plans for short-term and long-term workforces are studied carefully; hiring, firing, setting up staff salaries and allowances and the deployment of staff responsibilities and accountabilities are taken as priorities under organizing functions of management. Training and development, setting rewards and incentives are also included in this area of management responsibilities.

There is also a setting up of rules to observe work performances which are added to managements responsibilities not to mention its setting up of sets of reprimands, penalties for actions uncalled for while the staff or employee is in the performance of his duties and responsibilities. Moreover, in organizing functions, the management establishes the internal organization structure of the organization which primarily focuses on coordination, control of tasks and flow of information within the organization (Even, 2005). The IV should have been rechecked an hour post initiation of infusion. Instructions should have been followed to the letter (Dodman, N. 2003). Had this been well-established and regulated, the incident about the IV infusion rate on the baby would not have happened. (c) Directing or Leading. This function is aimed at channeling the behavior of all personnel to accomplish the organization's mission and objectives while simultaneously helping them accomplish their own career objectives (Even, 2005).

In directing or leading function of management, the management has the influence on the staffs or the employees behavior in the organization through motivation, communication, group dynamics, leadership and discipline (Even, 2005). In this case, nurses working with infants, especially in the Neonatal Intensive Care Unit must be competent, informed and skilled in the identification and management of hyperglycemia and hypoglycemia of the newborn to prevent morbidity and mortality. (Lagercrantz & Slotkin 1986). The nurses assigned to the baby in this situation should have been knowledgeable on the physiologic processes that regulate glucose production and utilization. This needs the right leadership qualities of everyone concerned.

The nurse on duty should have delegated the babys situation well so that there could not be any miss on the part of the next nurse on duty. Moreover, the next nurse should be equally competent to handle the situation and report any changes in the babys condition. As what happened in this critical incident, the was a big time gap before the incident was reported. What happened to the leadership qualities of the Head Nurses here? (d) Controlling. Controlling is a management function that is accomplished through a four-step process of establishing performance standards based on the companys objectives, measuring and reporting actual performance, while taking corrective and preventive actions as necessary (Even, 2005).

In this critical incident case, there could have been proper monitoring and documentation on the status of the baby. These interventions by the nurses are vital. There should have been a flow chart that prompts the nurses to review pertinent data and check the babys condition every now and then. Attention needed to be paid to thermal management. Knowledge on this will go a long way to keep the baby well. In summary, common to all health organizations are internal traits such as objectives, structure, processes, and behavior.

Furthermore, these traits are not separate and distinct. The different principles of planning of management come into play and are overlapping, interdependent factors in the broader organizational system.
