Plan of Care of a Movie "Stepmom" Family relationships are closely connected with self-care behavior. According to Orem, if self-care is well-performed, it helps to carry on human functioning, contributes to human development and maintains human integrity. (Orem, 1991). Generally, the concept of self-care behavior is defined as a process whereby people function on their own behalf in health promotion and prevention, and in disease detection and treatment. (Parissopoulos S. , Kotzabassaki S. , p. 3). Orem frequently uses the concept of self-care behavior in her theory of nursing, which consists of the three related ones: the theory of Self-Care, the theory of Nursing Systems, and the theory of Self-Care Deficit. In her theory of nursing, Orem views an individual as a self-care agent who possesses self-care agency (or capabilities), which is important to perform self-care actions. These actions are undertaken by the individual to meet the demand, which arises out of his / her needs for care.

If the demand is not met, the individual exercises self-care deficit, which requires qualified nursing. In my essay I will apply Orem's theory of Self-Care to the movie Stepmom and produce my own plan of care of one of the characters (Jackie). In order to obtain this purpose, I will assess the basic conditioning factors, examine self-care requisites, determine self-care deficit and work out my diagnosis of it. According to the plot of the movie Stepmom, the family of Jackie and Luke Harrison broke and the spouses divorced. Though after the divorce, Jackie and Luke managed to preserve fairly positive relationships because of their need to share their time with children - Ben and Annabelle, who were dear to them both. The conflict in the movie posed when Luke fall in love with Rachel, his girl-friend, and made an attempt to substitute Jackie with her.

Jackie and her children did not like Rachel in the role of stepmom, they demonstrated highly negative attitude to her. Despite Rachel's desperate endeavors to adopt her new role and gain kids love and friendship, she failed and showed her helplessness in family relationships. The core of the family conflicts and Rachel's inability to manage with children was Jackie. Jackie hated Rachel because: 1) Jackie was still jealous about Luke, 2) Jackie feared that her children would love Rachel more than her, 3) she loved her family and feared that she would be abandoned by it if Rachel becomes Lukes wife. Jackie's inadequate conduct and her negative treatment of Rachel were crucial for family relationships and family well-being. First, they affected the most vulnerable - children.

Ben appeared to be isolated and Annabelle had problems with social interaction at her school. Second, Rachel nearly lost her job because of her continuous troubles with kids. Third, Jackie tensed relationships between Luke and Rachel, which caused numerous quarrels between them. All in all, Jackie's self-care behavior was dangerous for the whole family and obviously requires nursing care. Making an assessment of basic conditioning factors, I can say that Jackie is a female in her early forties, who is diagnosed of cancer. Divorce (as Jackie's marital and social failure) and cancer (as her physical failure) are the two factors, which influence Jackie's self-care behavior and cause her self-care deficit.

Jackie's self-care requisites can be estimated as the following: - universal self-care requisites: reduced nutrient supply, alterations in her habitual activity and rest, - developmental self-care requisites: cancer as her major diagnosis and this situation results in Jackie's fear, anxiety and insecurity, - health-deviation requisites: inadequate attitude, denial to inform her family about her disease. Assessment of Jackie's self-care agency: because of her negative experience (cancer, divorce), Jackie finds herself in a new situation, where she feels lost and isolated, and where her new agencies are not developed yet. Diagnosis of the self-care deficit: Jackie has difficulty in adopting her new role in the family: divorced and distant wife, second mom; also she fears to become alone and to die. Jackie is unable to care for herself in her new situation, she is disoriented.

Plan of Jackie's care. Jackie's disorientation is related to her divorce, illness and her probable death. It is evidenced by: Major - desire for more contact with her family (Jackie shifts her days all the time in order to spend more time with children); desire to regain family attention to her (Jackie applies Mascara when she goes to the restaurant with Luke, Jackie presents Annabelle an expensive camera, Jackie tries to be Queen of Cool for her children, while Rachel remains only Princess); expressed feeling of melancholy and abandonment: she asks Ben his permission to call him when she feels blue. Minor - inability to concentrate and make a right choice: I can never figure out weather. Last trip, I made all the wrong choices; doubts about her ability to survive her disease.

Plan and outcome: Jackie will: identify the reasons of her fears and isolation; identify the ways to better the relationships she considers meaningful; identify the diversionary activities. Nursing interventions: - Jackie will be encouraged to verbalize her feelings and fears; - Jackie's nurse will help her to identify contributing and causative factors; - Jackie will be assisted to eliminate or diminish already known contributing and causative factors; - Jackie's nurse will assist her to identify the ways to better the relationships she considers meaningful; - Jackie's nurse will assist her to identify the diversionary activities. Thus, in my essay I have used Orem's theory of Self-Care to examine Jackie's (one of the characters in the movie Stepmom) self-care behavior. I have assessed the basic conditioning factors of Jackie, listed the three kinds of her self-care requisites: universal self-care requisites, developmental self-care requisites and health-deviation requisites. I have also determined Jackie's self-care deficit and worked out a number of diagnoses: a difficulty in adopting Jackie's new role in her family, fears of loneliness and death, disorientation. Finally, I have produced my own plan of care of Jackie, where I studied the evidences of Jackie's prioritized diagnosis and proposed plan of her nursing, which consists of her Jackie's actions and the actions of her nurse.
