One Flew Over The Cuckoos Nest: Antisocial Personality Behavior Diagnosis and Treatment The main character, Randle Patrick Mc Murphy, is brought to a state mental institution from a state prison to be studied to see if he has a mental illness.

Mc Murphy has a history of serving time in prison for assault, and seems to take no responsibility for his actions. Mc Murphy is very outgoing, loud, rugged, a leader, and a rebel. Mc Murphy also seems to get pleasure out of fighting the system. Mc Murphy relishes in challenging the authority of Nurse Ratchett who seems to have a strong hold over the other patients in the ward.

He enters into a power struggle with Nurse Ratchett when he finds out that he cannot leave the hospital until the staff, which primarily means her, considers him cured. As he tries to conform to the ways of the hospital, he actually becomes more like the patients that he detests. In one last attempt to escape from the hospital, Mc Murphy uses his cunning wit and skills as a con man to persuade the orderly into opening a window to allow two women into the ward. As the nigh progresses and he has the perfect opportunity to flee he realizes the hospital is the only safe place to stay. Due to Randal Mc Murphy rebellious ways and non-conformist ideas, the hospital performs a lobotomy, which during the time of this movie, was one of the only treatments known to suppress antisocial personality behavior. Antisocial personality behavior in a person is described as someone who has little disregard for and violation of the rights of others and is indicated by three or more of the following: ?

Failure to conform to social norms with respect to lawful behavior as indicated by repeatedly performing acts that are grounds for arrest. ? Deceitfulness, as indicated by repeated lying, use of alias, or conning others for personal profit or pleasure. ? Impulsivity or failure to plan ahead. ? Irritability and aggressiveness, as indicated by repeated physical fights or assaults. ? Consistent irresponsibility, as indicated by repeated failure to sustain consistent work behavior or honor financial obligations. ? Lack of remorse, as indicated by being indifferent to or rationalizing having hurt, mistreated, or stolen from another.

It is hard to first distinguish someone with this disorder because most of them are very friendly, outgoing, and have very likable personalities. Since they often feel no guilt or remorse they repeatedly leave behind a trail of friends, family, and unknown victims with a sometimes-irreversible damage of lies, manipulation, frustration and broken promises. The lack of moral or emotional development gives an antisocial person a lack of understanding for other peoples feelings. This enables them to be malicious and deceitful without feeling remorse about their actions.

They are incapable of having long lasting, close, warm and responsible relationships with people. The adult will also habitually lie and cannot hold a job for long periods of time. Studies have shown people diagnosed with antisocial personality behavior that are institutionalized seem to flourish under rigorous structure. In this organized setting very few privileges are given, but will be rewarded on the progress of their behavior. Since antisocial personalities thrive on hurting and using those around them, they must be educated in a new way of thinking. The goal is to teach them (1) how to trust others, (2) to share their feelings with others, (3) give them a new level of self-esteem, and (4) to be able to recognize when their actions are harmful to others.

Then there are those that are convicted of crimes and during their time of incarceration seem to do well under the strict rules of the institution, but since they usually do not receive any treatment the same behavioral patterns arise after a short time of release. Treatment with medication has not been proven beneficial, although some patients with antisocial personalities are being diagnosed with Attention Deficit Disorder (ADD) and treated with anti-suppressant drugs like Ritalin. There have been no studies of treatment with anti-depressants even though many antisocial personalities seem to encounter some kind of depression during their life. In dealing with antisocial personality behavior the three types of out patient therapy include individual, group and family. The first is Individual therapy.

In this specific style, the therapist tends to have the most success in gaining knowledge of the patients feelings of inadequacy, fear of intimacy, and low self esteem. Group therapy is a setting among other antisocial personalities. This style allows the patient a different type of incentive to improve some of their disorders. Lastly, there is family therapy, and in my opinion the most important. This is essential for both the patient and members of their family to understand and learn how to cope with this behavior. This style of therapy will teach family members not to be co-dependent and allow the patient to take responsibility for their actions.
