Fat cells lie beneath the skin throughout the body, but accumulate in greater amounts in certain areas. When we gain or lose weight, we are not adding or subtracting fat cells but rather changing the amount of fat within each cell. It is not surprising, therefore, that many people find that in spite of the weight they lose, they are left with bulges or fat deposits in undesirable places. Because of the frequency with which this operation is carried out, technical refinements have allowed consistently satisfactory results which are considerably better than was possible twenty-five or fifty years ago.

Nevertheless, patients must recognize that the goal of this operation is improvement, and that perfection is a goal that remains elusive. Patients who expect too much will be disappointed. Reshaping may involve a surgical procedure or may be accomplished with injection therapy, depending on the desired results. For overly large lips, excess tissue is removed surgically through carefully placed incisions in the inside of the lips. The incision is closed with a fine absorbable suture that does not require removal. Complications are uncommon with this operation but could include infection, wound separation or unsatisfactory scarring. We have not seen these complications in our experience. When done by itself, the surgery is easily carried out under local anesthesia. Scar Revision The effect of facial scarring is more than skin deep. Often it is very difficult to evaluate the emotional impact of such injuries. This is especially true with children who are unable to verbalize their feelings about the scars, or in males who are taught to minimize their concern about their appearance.

It is apparent that many people undergo unnecessary deformity because either they or their families have failed to deal with their concern over the effects of facial scarring or have failed to seek advice on what can be done to improve the appearance of facial scars. The treatment of facial scarring can be the most gratifying thing that a facial plastic surgeon does. On the other hand, it would be unfair if we did not point out that it is also one of the most difficult and challenging aspects of this type of practice. Unlike most cosmetic procedures, incisions usually cannot be hidden. The area of incision has already been predetermined by the injury. Often it is in the worst possible place, such as on the cheeks or on the jawline.
